工资 forms may be printed from this web site. Please click on the desired form and print a copy. Fill out the appropriate information. Mail or hand deliver the form to Frostburg State University's 工资 Office. Please remember that the contractual and regular payrolls are two separate systems. If you are on both payrolls, forms have to be done separately for each payroll respectively.
- Direct Deposit form (PDF)
Remember the following:
- Direct deposit will take at least two pay periods to take effect.
- Cancellation of an old account will occur within 21 days of receipt at Central 工资 Bureau; you will receive one or two payroll checks until the new account is established. Do not close the old bank account until a payroll check is issued.
- Discontinuation of direct deposit will occur within 21 days. Do not close your bank account until a payroll check is issued.
- Direct Deposit form must be typed and signed by hand in blue or black ink.

The i - 9形式 is used to verify that an employee is eligible to work in the United States. Completion of the form should be done prior to the employee's start date. Section 1 of the i - 9形式 is to be completed by the employee. For a student employee, section 2 should be completed by the supervisor or by an employee in the 工资 Office. Student employee's i - 9形式s are sent to the 工资 Office. Contingent I and Contingent II employee's forms are housed in the Office of Human Resources.
